A useful incident review is short, factual, and action-oriented.

Suggested Structure #

  1. Incident summary
  2. Customer impact
  3. Timeline (UTC)
  4. Root cause and contributing factors
  5. Mitigations completed
  6. Follow-up actions with owners and due dates

Common Pitfalls #

  • Too much narrative, not enough decisions
  • Missing owners for follow-up work
  • No severity-to-response mapping

If follow-up actions are not visible in planning systems, incident learning rarely turns into reliability gains.
